Written requests are welcome
The Village supports transparent government and access to public records under the Illinois Freedom of Information Act. You may use a request form, but any written request containing your contact information and a description of the records you want is accepted. Address your request to the Freedom of Information Act Officer.
How to submit your request
These instructions present the Village's published FOIA policy in webpage text. The original policy and request forms are available below. Be specific about the records you wish to inspect or receive; FOIA provides access to records rather than requiring answers to questions.

Send your written request to Sarah Witbracht at Sarah.Witbracht@peoriaheights.org, mail it to 4901 N. Prospect Road, Peoria Heights, IL 61616, or deliver it to the upstairs administration office at Village Hall. The policy also lists fax 309-686-2379.
Police records
The Village policy lists Rusty Weber or Shane Murphy as contacts for police requests. Deliver your request to the Police Department at 1311 E. Sciota Avenue, Peoria Heights, IL 61616, or fax it to 309-686-2384. You can also use the existing online police-records request form linked below.
